Your First Visit

Brief Paperwork
Upon entering the office, there will be some brief paperwork for you to fill out. The paperwork provides us with general information about you and your condition. (Download Patient Forms). Note: If your condition is related to a motor vehicle accident/personal injury or workers compensation claim, please note that there will be additional forms for you to complete when you arrive.

Consultation
Once the paperwork has been completed, you will have a consultation with the doctor. Dr. Brolin will take a complete patient history and discuss your health-related problems, concerns or injury. She will also discuss with you specific lifestyle factors that may be contributing to your problem and your health concerns.

History and Examination
In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a thorough orthopedic, neurological, and chiropractic examination will be performed to determine the nature of your condition, and the appropriate course of treatment.
